GENERAL SUMMARY:

The Lead Case Manager provides essential oversight and direction to Program Case Managers concerning the daily operations and systems integral to the Level 2 In Home Monitoring program. This role also entails delivering personalized case management services to a select group of youth and families involved in the pre-disposition and/or post-dispositional court processes within Milwaukee County's Division of Youth and Family Services.


KEY RESPONSIBILITIES:
  • Conduct training and coaching for new Case Managers on the policies and procedures of the Level 2 In Home Monitoring program.
  • Assist the Program Manager in preparing necessary reports, data collection tools, and the intake and assignment of cases.
  • Implement flexible, non-routine tracking to ensure youth comply fully with court orders and curfews.
  • Demonstrate a commitment to working with any youth and family referred for the duration of their involvement in the program, emphasizing their strengths, culture, and community.
  • Support the family decision-making process to empower youth and families to prevent further legal violations or continued involvement in the Juvenile Justice System.
  • Ensure all documentation is prepared according to the program's designated timelines.
  • Present reports to contracting entities, including court officials.
  • Engage professionally with school staff to monitor daily attendance, address behavioral issues, and enhance the performance of each youth to improve their functioning and behavior.
  • Coordinate and fulfill assigned tracking and coverage per program expectations, ensuring mandatory counseling hours and recreational activities are provided to each youth on the caseload.
  • Facilitate, counsel, and supervise youth and family group sessions effectively.
  • Provide Global Positioning System (GPS) monitoring of youth in accordance with program expectations.
  • Be cross-trained in all Community Monitor functions.
  • Adhere to St. Charles Youth & Family Services Policies and Procedures.
  • Comply with all Level II Case Manager Functions.
  • Perform other duties as assigned.

MINIMUM QUALIFICATIONS:
  • Bachelor's degree in Human Services, Social Work, Criminal Justice, or a related field.
  • Two years of experience with programs serving delinquent or at-risk youth populations.
  • One year of experience with the targeted population (Level 2 program preferred).
  • Valid driver's license and liability insurance.
  • Ability to utilize vehicles for work-related duties in accordance with agency insurance liability standards.
  • Demonstrated cultural competence by responding respectfully and effectively to individuals from diverse backgrounds.
  • Knowledge of adolescent and family issues and dynamics.
  • Appreciation for cultural and socioeconomic dynamics.
  • Understanding of human growth and development theories.
  • Proficiency in accurate record-keeping and documentation.
  • Competence in performing computer tasks using MS Windows applications and agency database systems.

WORKING CONDITIONS:
  • Indoor and outdoor requirements, including client homes and community settings.
  • Potential for physical intervention with clients as necessary.
  • Ability to lift a maximum of 50 lbs.
  • Local travel may be required to fulfill essential functions of the position.
