Core Responsibilities:

· **Client Engagement & Advocacy:** Deliver personalized support, providing knowledgeable guidance and assistance in formulating customized service plans.

· **Case Oversight:** Keep precise documentation, assess client needs, and manage a caseload of active participants.

· **Post-Release Support:** Track client progress, provide individualized support, and lead group discussions during critical reentry stages.

· **Reentry & Employment Assistance:** Organize essential resources, oversee financial matters, aid in benefits acquisition, and collaborate with employment navigators.

· **Resource Management:** Address needs related to housing, transportation, employment, and counseling, offering emergency financial assistance when feasible.

· **Collaboration:** Connect with reentry service providers and community organizations to link clients with essential resources.

Qualifications:

· Availability for full-time work, with the flexibility to accommodate evenings and weekends.

· Proficient in Microsoft Office and case management systems.

· Strong organizational and record-keeping capabilities.

· Familiarity with case management protocols and substance use challenges.

· Awareness of barriers to workforce entry and problem-solving strategies for individuals reintegrating into society.

· Ability to prioritize effectively while demonstrating compassion and patience.

· Exceptional communication and presentation skills.

· An associate degree is preferred; relevant reentry experience may be considered as a substitute.

· Previous experience in case management is preferred.

· Valid Driver's License and dependable transportation are required.

Work Environment:

· Prolonged use of computers.

· Adaptability to non-traditional office settings, including correctional facilities.

· Willingness to work occasional nights and weekends.

· Capacity to manage stress and interpret verbal communication effectively.
