Senior Case Manager

Position Overview: The role involves delivering continuous service coordination, crafting treatment strategies, advocating for, and monitoring individuals residing in designated counties who are engaged in various rehabilitation programs.
Key Responsibilities:
- Facilitate, organize, and ensure effective follow-up with community-based treatment for individuals enrolled in rehabilitation programs.
- Establish and nurture a supportive therapeutic rapport with each client.
- Integrate evidence-based methodologies into service delivery, including but not limited to Motivational Interviewing (MI), Trauma-Informed Care (TIC), and Person-Centered Planning.
- Employ MI techniques as a primary strategy to enhance client engagement and success.
- Oversee ongoing treatment requirements and adherence for all program participants by conducting assessments, developing treatment plans, and providing assertive case management, symptom management, medication oversight, crisis intervention, and coordination of multidisciplinary team meetings as per established protocols.
- Accountable for preparing and submitting necessary legal documents for program participants as mandated.
- Coordinate team meetings with relevant stakeholders and treatment team members.
- Uphold safety practices and continuously evaluate potential risks while working in the community.
- Facilitate access to community support services and assist clients with daily living activities, including coaching and hands-on support in areas such as housing, financial management, vocational/educational goals, and transportation.
- Encourage clients to build a natural support network involving family, friends, and community members.
- Create and maintain comprehensive service documentation, including case notes, assessments, and various reports within specified timeframes.
- Adhere to program and quality standards, ensuring compliance with documentation protocols.
- Maximize client financial contributions and third-party payments for service costs by exploring insurance benefits and community resources.
- Maintain ongoing communication and collaboration with treatment providers and other relevant agencies, ensuring timely responses to communications.
- Conduct regular home visits and provide transportation for clients as necessary.
- Engage in continuous professional development to enhance MI skills and achieve fidelity within the designated timeframe.
- Participate in required training sessions and complete necessary assessments.
- Foster positive relationships with stakeholders and funding entities.
Additional Responsibilities:
- Timely collection of essential medical records and participant information.
- Participate in crisis line coverage as needed.
- Attend court hearings as required.
- Engage in training, meetings, and staff collaboration.
- Participate in staff shadowing initiatives.
- Other related duties as necessary to fulfill job responsibilities.
Qualifications:
- Bachelor's Degree in a relevant human services field.
- Experience in case management and service coordination.
- Experience working with individuals facing mental health challenges or substance use disorders.
- Valid driver's license and access to a reliable vehicle.
- Ability to pass background checks as required.
Work Environment:
This position involves a combination of office and fieldwork across multiple counties, engaging with individuals in various rehabilitation programs. The role requires travel to different facilities and involves direct interaction with clients in their living environments.
Physical Requirements:
The role necessitates frequent driving and the ability to assist clients with physical tasks as needed. Office work includes sitting at a desk and performing computer-related tasks.
