Position Summary: Targeted Case Manager

To support our case managers in achieving a healthy work/life balance, we offer a flexible hybrid work schedule, allowing them to work from home, the office, and within the community.

Qualifications:

Applicants must possess a valid driver's license, vehicle, and auto insurance, along with a Bachelor's degree (B.A. / B.S.) from an accredited institution. A minimum of one year of relevant experience or training is required, or an equivalent combination of education and experience.

Key Responsibilities:
  • Stay informed about community resources.
  • Conduct comprehensive assessments.
  • Develop individualized recovery plans (IRPs) and ensure they are reviewed biannually.
  • Continuously monitor the effectiveness of recovery plans.
  • Provide strengths-based support to clients.
  • Build and maintain effective relationships with community partners and stakeholders.
  • Offer crisis case management as necessary.
  • Maintain an empathetic, trauma-informed approach.
  • Manage cases efficiently within authorized units.
  • Occasionally transport clients to appointments.
  • Assist in transitioning clients to less restrictive environments.
  • Monitor medication effectiveness.
  • Ensure safe and affordable housing options.
  • Support clients in community integration.
  • Complete time-sensitive documentation promptly.
  • Assist clients in pursuing employment and educational opportunities.
  • Coordinate with relevant medical providers.
  • Uphold confidentiality in accordance with HIPAA regulations.
  • Participate in required training and meetings.
  • Support clients with financial management and social security applications.
  • Act as an advocate for clients within the community.
  • Assist clients with relocation and grocery shopping.
  • Attend client events and agency initiatives.
  • Engage in agency improvement projects.
  • Perform additional duties as assigned.
