Key Responsibilities- Case Management: Maintain a caseload of up to 30 residents, providing individualized counseling and support to help them achieve treatment goals.
- Documentation: Complete written work on time, including social history, LS/CMI assessments, individualized program plans, discharge summaries, and required reports, in accordance with regulatory and funding source guidelines.
- Collaboration: Schedule and attend meetings with United States Probation Officers, the Bureau of Prisons RRM Office, and Community Treatment Services (CTS) to ensure seamless coordination of services.
- Staff Meetings: Attend regular staff meetings to stay informed about program developments and share knowledge with colleagues.
- Progress Monitoring: Regularly monitor residents' progress toward individualized program plan goals and hold them accountable to established guidelines.
- Referrals: Refer residents to collateral services as needed, ensuring they receive comprehensive support.
- Training: Attend mandatory trainings to enhance knowledge and skills in correctional counseling and case management.
- Additional Duties: Fill in for absent staff, assist in training new staff, and perform other duties as assigned.
- Education: Bachelor's degree in a social service field.
- Experience: Minimum of two years' counseling experience, preferably in a correctional setting.
- Certifications: Clear WI Caregiver Law, pass State and Federal Criminal Background Check, and pre-employment health and drug screening.
- Transportation: Valid Driver's License, access to a reliable vehicle, and auto insurance sufficient to meet agency requirements.
The Case Manager II will work in a busy residential setting, interacting frequently with residents, colleagues, Probation/Parole staff, outside professionals, and visitors. The environment requires the ability to climb two to three flights of stairs, perform CPR, and respond to emergency situations.
