Key Responsibilities:
- Facilitate, strategize, and ensure effective follow-up with community treatment services for individuals engaged in re-entry and competency restoration programs.
- Establish and nurture a supportive therapeutic rapport with each participant.
- Integrate evidence-based methodologies into service delivery, including but not limited to Motivational Interviewing (MI), Trauma-Informed Care (TIC), and Person-Centered Planning.
- Employ MI techniques to enhance participant engagement and success.
- Assess ongoing treatment requirements and adherence for all program participants, providing comprehensive case management, treatment planning, and crisis intervention as per established protocols.
- Responsible for drafting and submitting necessary legal documents for participants as mandated.
- Coordinate multidisciplinary team meetings with relevant stakeholders.
- Maintain safety protocols and continuously evaluate potential risks while working in community settings.
- Facilitate access to community support services and assist participants with daily living activities, including housing, financial management, and vocational pursuits.
- Encourage the development of a natural support network for each participant, involving family, friends, and community resources.
- Create and manage participant service documentation, ensuring compliance with program timelines and quality standards.
- Maximize participant financial contributions and third-party payments by leveraging available benefits and community resources.
- Maintain ongoing communication with treatment providers and relevant agencies, ensuring timely responses to inquiries.
- Conduct regular home visits and provide transportation for participants as needed.
- Engage in continuous professional development through training and coaching in MI techniques.
- Participate in required assessments and training sessions as stipulated by program guidelines.
- Build and maintain positive relationships with stakeholders and funding organizations.
Additional Responsibilities:
- Timely collection of essential medical records and participant information.
- Participate in crisis line coverage as needed.
- Attend court hearings as required.
- Engage in training sessions and team meetings.
- Participate in staff orientation and training.
- Other related duties as necessary to fulfill job responsibilities.
Qualifications:
- Bachelor's Degree in a relevant human services field.
- Experience in case management and service coordination.
- Experience working with individuals facing mental health challenges or substance use disorders.
- Valid driver's license and access to a reliable vehicle.
- Ability to pass background checks as required.
Work Environment:
The position involves a mix of office work and field engagement across multiple counties, requiring travel to various facilities and community locations. Staff members are expected to transport participants as necessary.
Physical Requirements:
The role necessitates frequent driving and the ability to lift and carry items as needed to assist participants.
