Position: Primary Care Provider
Role Overview:
- Work alongside a committed care team to tackle social determinants affecting the health of older adult patients.
- Support patients in reaching their personal health objectives by providing comprehensive and high-quality care.
- Leverage advanced technology for both in-person and telehealth consultations to establish lasting, trusted care relationships.
- Promote individual and community wellness through a patient-centered approach.
- Engage in daily team meetings to address patient concerns and formulate care strategies.
- Perform in-person assessments and telehealth consultations, with assistance from EMTs in patient residences.
- Collaborate with Gather Guide Partners to meet patients' social needs.
- Effectively communicate with emergency room staff regarding patient histories and care plans.
- Provide ongoing support and follow-up for patients throughout their health journeys.
- Document patient visits and clinical notes in our electronic health record system (Athena).
- Offer on-call support for complex cases.
- Board Certified Nurse Practitioner with valid licensure in Massachusetts.
- Expertise in evidence-based primary care practices.
- Ability to connect with patients from diverse backgrounds.
- Experience with electronic health record systems and relevant technologies.
- Strong ethical standards and confidentiality in managing sensitive information.
- Commitment to mission-driven healthcare and community enhancement.
- Collaborative spirit and capability to thrive in a non-hierarchical setting.
- Flexibility and composure in managing competing priorities.
- Authorized to work in the United States.
- Bilingual in Spanish is preferred.
- Experience working with senior populations in underserved communities facing social challenges.
