Key Responsibilities- Direct Patient Care: Provide high-quality nursing care to patients, including assessments, care planning, and implementation.
- Interdisciplinary Collaboration: Work closely with the interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each patient.
- Communication and Coordination: Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Medication Management: Ensure accurate and timely medication administration, including ordering, stocking, and monitoring.
- Education and Consultation: Provide education and consultation to patients and caregivers on care plans, nursing treatments, and medications.
- Team Leadership: Assist the Charge Nurse in leading the team, including training, oversight, and workflow management.
- License and Certification: Current Registered Nurse license in the Commonwealth of MA.
- Experience: Minimum of two (2) years of experience working with the geriatric population.
- Skills: Proficient in Microsoft Office tools and able to travel locally on a frequent basis.
