Job Summary

Element Care is seeking a highly skilled Nurse Practitioner to join our interdisciplinary team as a Geriatric Care Specialist. This full-time position will provide comprehensive primary care to our participants in a long-term care facility setting.

Key Responsibilities
  • Conduct Initial Examinations and Quarterly Evaluations: Conduct thorough initial examinations of new participants and quarterly evaluations thereafter, for the first year of enrollment.
  • Evaluate Participants for Episodic Visits and Acute Illnesses: Evaluate participants for episodic visits, acute illnesses, and follow-ups of hospitalizations.
  • Provide Health Care Teaching and Education: Provide appropriate health care teaching to patients, their families, and caregivers, as well as to other members of the interdisciplinary team.
  • Collaborate with Medical Doctors and Interdisciplinary Team: Collaborate with medical doctors at the site to provide comprehensive primary care to participants.
  • Participate in After Hours On-Call Coverage: Participate in after-hours on-call coverage to ensure timely and effective care.
  • Document Patient Information Accurately and Timely: Complete accurate, timely documentation, onsite and offsite, to ensure seamless communication and care.
  • Ensure Fiscally Responsible Practices: Ensure that duties are accomplished in a fiscally responsible manner, adhering to established budgetary constraints.
  • Adhere to Highest Standards of Care: Ensure that all interactions with clients are conducted in a manner that adheres to the highest standards of care for all Element Care clients.
Requirements
  • Master of Science Degree in Nursing (MSN): Possess a Master of Science Degree in Nursing (MSN) from an accredited institution.
  • Current Board Certification as a Nurse Practitioner: Hold current board certification as a Nurse Practitioner in the Commonwealth of Massachusetts.
  • Massachusetts and DEA Prescription Privileges: Possess Massachusetts and DEA prescription privileges.
  • Credentialed for Long Term Care Facilities: Be credentialed for the Element Care affiliate long-term care facilities.
  • Current CPR Certification: Hold current CPR certification.
  • Minimum of One Year Working with Geriatric Population: Possess a minimum of one year working with a geriatric population.
  • Ability to Communicate Effectively: Demonstrate the ability to convey information clearly and succinctly with all employees, so that relevant information is communicated appropriately and in a timely manner.
  • Ability to Lead and Work Well with Others: Demonstrate the ability to lead independently and to work well with others, considering the needs of colleagues and team members.
  • Ability to Pay Attention to Detail: Demonstrate the ability to pay attention to detail, be a self-starter, and complete tasks on time.
  • Ability to Recognize and Implement Effective Solutions: Demonstrate the ability to quickly recognize situations/problems before they become acute, and to implement effective solutions and understand consequences.
