Job Summary

We are seeking a skilled and compassionate Home Care Coordinator to join our team at Element Care. As a key member of our PACE (Program of All-Inclusive Care for the Elderly) team, you will play a vital role in delivering high-quality care to our participants.

Key Responsibilities
  • Assessment and Care Planning: Conduct initial assessments of participants in their home environments to determine their individual needs and coordinate their initial care plans.
  • Home Visits and Reassessments: Perform annual in-home reassessments of all participants to determine what services are needed and appropriate, and provide home visits on an as-needed basis to triage changes in medical, cognitive, or functional status.
  • Care Planning and Coordination: Offer solutions to revise or modify care plans to better meet the needs of participants and participate in the care planning process.
  • Documentation and Communication: Record and maintain accurate and timely documentation in participants' charts, and effectively communicate with participants and families regarding home care services.
  • Team Collaboration: Participate in family meetings or case conferences as necessary, and collaborate with the interdisciplinary team to ensure seamless care delivery.
  • Quality Assurance: Perform monthly chart reviews and annual assessments to ensure compliance with regulatory requirements and maintain high-quality care standards.
  • Additional Responsibilities: Provide rotating coverage for after-hours on-call system, ensure that all interactions with participants, families, and vendors are conducted in a manner that adheres to the highest standards of care and professionalism, and perform other duties as assigned.
Requirements
  • Education and Licensure: Successful completion of an accredited R.N. program, Bachelor of Science degree in Nursing (preferred), and current licensure as a Registered Nurse in the Commonwealth of Massachusetts.
  • Experience: A minimum of 2 years of experience working with a geriatric population and 2 years of working in a community health setting.
  • Certifications: Current CPR certification.
