Job Summary

We are seeking a highly skilled and compassionate Primary Care Nurse to join our team at the Lynn Community Health Center. As a Primary Care Nurse, you will play a vital role in providing high-quality patient care and promoting health education to our community.

Key Responsibilities
  • Triage patients by telephone or in person to determine urgency and level of care needed, ensuring appropriate care disposition.
  • Perform patient procedures, assess patient health status, and provide health education to patients/families regarding pertinent health issues, clinical conditions, treatments, recommended follow-up actions, and/or home care.
  • Communicate pertinent abnormal test results to providers.
  • Track patients for necessary follow-up, documenting appropriately.
  • Utilize your skills to interact with patients/clients, team members, and staff from other departments.
Requirements
  • Graduate of an accredited nursing program, current Mass. licensure, BLS certification.
  • Bilingual in Spanish/Khmer/Russian with knowledge of medical terminology preferred.
  • Previous Community Health or Ambulatory Experience preferred.
  • A desire to work as a collaborative member of a multi-disciplinary team.
