Advanced Practice Provider

We are looking for a dedicated full-time or part-time Nurse Practitioner (NP) or Physician Assistant (PA) to provide exceptional patient care within a primary care environment in Maryland.
The NP/PA will be responsible for delivering both preventive and acute care in an office and skilled nursing facility (SNF) setting. Accurate clinical documentation will be maintained in our electronic health record (EHR) system, and the provider will utilize Accountable Care Organization (ACO) tools to facilitate patient access to high-quality healthcare services.
Prior experience is advantageous but not mandatory. New graduates are encouraged to apply.
Key Responsibilities:
- Deliver clinical primary care in both office and SNF settings, including conducting screenings, performing history and physical examinations, documenting care plans, administering vaccinations, and making high-value referrals.
- Improve the coordination of care by integrating records from acute, specialist, and post-acute visits into the clinical documentation, extracting valuable clinical insights.
- Ensure high-quality care delivery and maintain thorough documentation to achieve excellent quality scores in commercial and public payer ACO programs.
- Provide efficient patient care to reduce wait times and meet practice productivity objectives.
- Enhance patient satisfaction through evidence-based education, collaborative decision-making, and exemplary bedside manner.
