GENERAL OVERVIEW:

The RN Care Coordinator is responsible for delivering comprehensive case management services to designated SCO members facing intricate health challenges, ensuring effective and high-quality care. This role involves the formulation, execution, and assessment of personalized care strategies. The individual will comply with established healthcare standards and the policies of Element Care. This is a full-time position with a standard schedule of Monday through Friday, with no weekend or holiday obligations.

KEY RESPONSIBILITIES:

  • Formulates a care strategy that aligns with medical directives by evaluating the patient's requirements, health status, surroundings, and collaborating with other healthcare professionals during the initial assessment and regularly updating the care plan as needed, documenting all actions in accordance with Element Care protocols.
  • Ensures the provision of skilled and quality care within the home environment.
  • Maintains ongoing accountability for the assigned patient caseload.
  • Supervises the execution and assessment of individualized care plans.
  • Facilitates communication with Commonwealth Care Alliance (CCA) and Element Care personnel, as well as relevant healthcare providers.
  • Offers educational resources to patients.
  • Engages with both members and their families.
  • Actively participates in collaborative team knowledge-sharing initiatives.
  • Performs additional responsibilities as required.

QUALIFICATIONS:

  • Registered Nurse with a current license in the Commonwealth of MA.
  • Bachelor of Science in Nursing is preferred.
  • A minimum of three (3) years of experience as a licensed, registered nurse, ideally five (5) years, with a focus on home healthcare.
  • Proficient in Microsoft Office applications.
  • Experience with case management information systems.
  • Willingness to travel locally on a regular basis.
