Position Title: Primary Care Provider

Role Overview:
- Work collaboratively with a committed care team to address the social determinants affecting the health of older adult patients.
- Support patients in achieving their personal health goals by providing high-quality, holistic care.
- Leverage advanced technology for both in-person and telehealth consultations to build trusted, long-term patient relationships.
- Promote individual and community health through a patient-centered care approach.
- Engage in daily team meetings to discuss patient issues and care strategies.
- Perform in-person assessments and telehealth appointments, with assistance from EMTs in patient residences.
- Collaborate with Gather Guide Partners to meet patients' social needs.
- Communicate effectively with emergency department staff regarding patient histories and care plans.
- Provide ongoing support and follow-up for patients throughout their health journeys.
- Document clinical visits and notes in our electronic health record system (Athena).
- Offer on-call support for complex cases.
- Board Certified Nurse Practitioner with valid licensure in Massachusetts.
- Expertise in evidence-based primary care practices.
- Ability to connect with patients from diverse backgrounds.
- Experience with electronic health record systems and relevant technologies.
- Strong ethical standards and confidentiality in managing sensitive information.
- Commitment to mission-driven healthcare and community enhancement.
- Collaborative approach and capacity to work in a flat organizational structure.
- Flexibility and composure in managing competing priorities.
- Authorized to work in the United States.
- Bilingual in Spanish is preferred.
- Experience working with senior populations in underserved communities facing social challenges.
