Job Summary

Element Care is seeking a highly skilled and compassionate Nurse Practitioner to join our interdisciplinary team. As a Geriatric Care Specialist, you will play a vital role in providing comprehensive primary care to our participants, collaborating with Medical Doctors to ensure the highest standards of care.

Key Responsibilities
  • Conduct thorough initial examinations and quarterly evaluations for new participants and existing clients, respectively.
  • Evaluate participants for episodic visits, acute illnesses, and follow-ups of hospitalizations, ensuring timely and effective interventions.
  • Make nursing home visits as required by state regulations and as needed, prioritizing the well-being of our participants.
  • Provide home visits for acutely or chronically ill participants who are home-bound, demonstrating a commitment to delivering care in the most convenient and comfortable setting possible.
  • Participate in After Hours On-Call Coverage, ensuring seamless care delivery outside of regular working hours.
  • Deliver health education to patients, their families, and caregivers, as well as to other members of the Interdisciplinary Team, promoting informed decision-making and empowerment.
  • Adhere to the standards set forth in the Element Care NP/MD Collaborative Practice Guide, upholding the highest standards of care and professionalism.
  • Maintain accurate, timely, and comprehensive documentation, both onsite and offsite, to ensure seamless communication and continuity of care.
  • Ensure that all interactions with clients are conducted in a manner that adheres to the highest standards of care for all Element Care clients, prioritizing dignity, respect, and compassion.
  • Ability to pass a fit test, with a mask seal being critical; incumbent is required to not have facial hair that interferes with a tight seal of the respirator.
  • Frequent local travel may be required.
  • Perform other duties as assigned, demonstrating flexibility and a commitment to the team's success.
Requirements
  • Master of Science Degree in Nursing (MSN).
  • Current Board Certification as a Nurse Practitioner in the Commonwealth of Massachusetts.
  • Massachusetts and DEA Prescription Privileges.
  • Credentialed for the Element Care affiliate Long Term Care Facilities.
  • Current CPR Certification.
  • Minimum of one (1) year working with a geriatric population.
  • Ability to convey information clearly and succinctly with all employees, so that relevant information is communicated appropriately and in a timely manner.
  • Ability to lead independently and to be able to work well with others.
  • Ability to work within a team concept and to be considerate of co-workers and colleagues.
  • Ability to pay attention to detail, to be a self-starter, and to complete tasks on time.
  • Ability to be able to quickly recognize situations/problems before they become acute, and to be able to implement effective solutions and understand consequences.
  • Covid vaccine required.
