The Senior Patient Services Coordinator plays a vital role within the healthcare team, acting as the primary liaison for patients. Under general supervision, the Coordinator provides essential administrative support to healthcare professionals in a bustling outpatient practice. Collaborating closely with the Physician Assistant, the Coordinator ensures that Urologists receive the necessary assistance for their operations.

Key Responsibilities:

The Coordinator will be the main point of contact between the Urology care team and patients, supporting team members in all aspects of care coordination and management. Specific duties include:

  • Facilitating patient care services, including scheduling visits with specialists and arranging necessary tests and procedures.
  • Processing insurance referrals and prior authorizations as needed.
  • Documenting and tracking all required services within the care coordination system, ensuring timely completion and communication of urgent care items.
  • Maintaining proactive communication with patients regarding their care plans and any scheduling issues.
  • Monitoring clinical activities required by patients, including routine screenings and follow-up visits.
  • Engaging with patients who may not be compliant with their care plans to encourage adherence.
  • Coordinating with specialty offices to expedite care needs and gathering necessary pre-visit documentation.

In addition to care coordination, the Coordinator will:

  • Manage incoming calls, greet patients, and provide accurate information in a timely manner.
  • Document coordination efforts in the electronic medical record system.
  • Schedule patient appointments and coordinate ancillary services.
  • Identify barriers to care and advocate for patients to facilitate access to necessary services.
  • Act as a liaison among the healthcare team to ensure continuity of care.

The Coordinator will also maintain the administrative and clinical calendars for physicians, prepare correspondence, and manage surgical case preparations.

Active participation in team meetings and huddles is essential to update team members on care coordination efforts.

Qualifications:

  • Strong interpersonal communication skills, both verbal and written.
  • Exceptional patient care attitude and skills, with a focus on teamwork and process improvement.
  • Ability to work independently and collaboratively within a team.
  • Demonstrated organizational and time management skills.
  • Proficiency in Microsoft Office Suite and other relevant software.
  • Understanding of HIPAA confidentiality and privacy policies.

Education:

  • Minimum: High school diploma or equivalent.
  • Preferred: Some college or an Associate's Degree.

Experience:

  • 1-2 years of experience in a secretarial or customer service role within a medical or healthcare setting.
  • Preferred: Experience in a clinical or healthcare-related environment.
