Key Responsibilities:
The Coordinator will be the main point of contact between the Urology care team and patients, facilitating care coordination and management. Specific duties include:
- Coordinating patient care services, including scheduling appointments with specialists and arranging necessary tests and procedures.
- Managing insurance referrals and prior authorizations to ensure seamless service delivery.
- Documenting and tracking all required services within the care coordination system.
- Communicating effectively with patients and care team members regarding appointment statuses and any issues that arise.
- Monitoring patient compliance with care plans and proactively reaching out to those needing assistance.
- Facilitating communication between patients and healthcare providers to promote continuity of care.
- Maintaining accurate patient records and ensuring all pre-visit requirements are met.
Additional Duties:
The Coordinator will also:
- Answer and manage incoming calls, providing accurate information and directing inquiries as needed.
- Assist in the preparation for surgical cases, confirming necessary pre-operative testing and documentation.
- Participate in team meetings to share updates on patient care coordination efforts.
- Support the care team by managing incoming communications and ensuring timely responses to patient needs.
Qualifications:
- Strong interpersonal communication skills, both verbal and written.
- Demonstrated commitment to providing exceptional patient care and service.
- Ability to work independently and collaboratively within a team environment.
- Excellent organizational skills and the ability to prioritize tasks effectively.
- Proficiency in common office software applications.
- Understanding of HIPAA regulations and commitment to patient confidentiality.
Education and Experience:
- High school diploma or equivalent required; some college or an Associate's Degree preferred.
- 1-2 years of experience in a customer service or administrative role within a healthcare setting is preferred.
