Since its inception, the Boston Health Care for the Homeless Program has dedicated itself to providing equitable and dignified access to top-tier healthcare for individuals and families facing homelessness in the greater Boston area. Each year, we serve over 10,000 homeless individuals, ensuring they receive comprehensive healthcare services ranging from preventive dental care to advanced cancer treatment. Our dedicated team of clinicians, case managers, and behavioral health experts operates in more than 30 locations, striving to support some of the most vulnerable members of our community.

Our community-based healthcare services are designed to be compassionate, culturally appropriate, and responsive to the social determinants of health, aiming to eliminate both physical and systemic obstacles faced by our patients.

Position Summary:
This is a full-time role, operating Monday through Friday from 8:30 AM to 5:00 PM. In light of the pressing issues surrounding drug overdose among homeless individuals in Boston, we are expanding our initiatives to include treatment for stimulant use disorders. We are seeking a Healthcare Coordination Specialist to join our Recovery Team.

Key Responsibilities:

  • Manage the walk-in/Open Access schedule, ensuring patients are informed about their status and the plans being developed to meet their needs.
  • Provide recovery navigation services, assisting individuals in connecting with both internal and external support systems.
  • Oversee intakes, data collection, recovery rewards distribution, and engagement activities for our stimulant treatment program.
  • Support community outreach efforts, including educational events and health fairs.
  • Facilitate team meetings by coordinating schedules, taking minutes, and managing communications.

Qualifications:

  • Proven commitment to harm reduction and collaboration with individuals experiencing addiction or recovery; lived experience is highly valued.
  • Awareness of the challenges related to homelessness, poverty, and racism, and their impact on individuals with substance use disorders.
  • Experience in administrative roles, including database management and proficiency in software applications like Microsoft Excel.
  • Skills in customer service and managing patient flow, with an emphasis on alleviating anxiety and frustration.
  • Experience in navigating complex medical or social systems, such as case management or recovery support.
  • Ability to engage with individuals in distress and foster long-term supportive relationships.

Compensation and Benefits:

  • Starting compensation is $22.07 per hour, with increases based on experience.
  • Eligible full-time employees enjoy a competitive time-off policy, including 4 weeks of vacation, comprehensive health benefits, a retirement savings plan, and a pre-tax transportation program.
  • Additional compensation for demonstrated bilingual proficiency and opportunities for collaboration with local healthcare providers.
