Job Overview

Position Summary: The role of the Home Care Case Coordinator is essential in ensuring the smooth operation of client services in alignment with existing federal, state, and local regulations governing home care. This position provides guidance for comprehensive client case management while fostering new business relationships to enhance service delivery.

Key Responsibilities:

  • Community and Caregiver Engagement: Act as a role model for scheduling staff, participate in caregiver recruitment, and identify opportunities for client referrals.
  • Client Management: Ensure that all client needs are addressed, conduct in-home assessments, and develop tailored Plans of Care.
  • Communication: Maintain effective communication with clients, families, and caregivers through various channels including home visits and written documentation.
  • Problem Resolution: Collaborate with the team to address and resolve client and caregiver issues proactively.

Qualifications:

  • A Bachelor's degree in a health or human services field is preferred.
  • Minimum of two years of experience in home health care.
  • Strong interpersonal skills to interact effectively with clients and families.
  • Proficiency in Microsoft Office and electronic documentation.
  • Ability to manage stressful situations with empathy and patience.

Additional Information: This position requires flexibility in working hours, including evenings and weekends, and the ability to travel as needed.
