Position Summary: The Home Care Case Coordinator plays a vital role in ensuring the seamless operation of our services, adhering to all relevant federal, state, and local regulations governing home care. This position is essential for leading client case management efforts and fostering new business relationships to enhance service delivery.

Key Responsibilities:

  • Community and Caregiver Engagement: Set a standard for Scheduling Coordinators, participate in caregiver recruitment, and identify new client referral opportunities.
  • Client Interaction: Address new client inquiries and manage relationships to ensure their needs are met and expectations exceeded.
  • Assessment and Planning: Conduct in-home evaluations to identify safety risks and develop comprehensive Plans of Care tailored to individual client needs.
  • Communication: Maintain effective communication with clients, families, and caregivers through regular visits, meetings, and updates.
  • Team Collaboration: Work closely with team members to resolve issues and ensure high-quality care delivery.

Qualifications:

  • Bachelor's degree in a health or human services field is preferred.
