WellBe Senior Medical Careers is seeking a highly skilled and compassionate Senior Medical Care Specialist to join our team. As a key member of our interdisciplinary care team, you will play a vital role in delivering high-quality, patient-centered care to our geriatric patients.
Key Responsibilities- Model the WellBe values and mission in care delivery for our patients.
- Follow state and federal regulatory agencies and certified agencies for their region.
- Provide primary care in the home-based setting for patients.
- Engage new patients and ensure comprehensive medical records are completed.
- Partner with other clinicians to manage patient health and wellbeing.
- Facilitate staff, patient, and family decision-making through educational resources.
- Perform initial patient screenings/physical examinations and create assessment/plan.
- Order, interpret, and analyze diagnostic tests and determine follow-up appointments and treatment options.
- Prescribe and manage medications and treatments as appropriate.
- Update patient records and check for accuracy at each appointment.
- Deliver high-quality care while maintaining company metrics.
- Serve as a key resource for patients in delivering ongoing care information, counseling, and guidance.
- Collaborate with the interdisciplinary team and participate in regularly scheduled team meetings.
- Master's Degree in Nurse Practitioner studies from an accredited, master's-level NP program.
- Active, unencumbered Registered Nurse and Nurse Practitioner licenses in the posted state and must reside in state.
- Current board certification, either AANC or AANP.
- BLS certified and an active NPI# as an NP.
- CPR certification.
- Active, unencumbered DEA licensure or eligible to apply for upon hire.
- Previous experience in home care, primary care, hospice, palliative care, or geriatrics strongly preferred.
- Valid driver's license, car insurance, access to a car, and willingness to drive to patient locations.
- Ability to conduct in-home setting visits.
- Demonstrated role proficiency with Microsoft Office; Excel, Word, G-suite, etc.
- Proficient with office software, including Electronic Health Records, Scheduling Software, and Ring Central applications.
- Role model in courageous teamwork, integrity, ethics, and productivity.
- Experience in diagnosing and treatment of ongoing, chronic, and urgent medical/psychological conditions of older adults.
- Experience in panel management, primary care, geriatrics, internal medicine, palliative care.
- Experience in palliative care and end-of-life communication.
- Excellent verbal, written, presentation, and interpersonal communication skills.
- Confident speaker to internal and external groups.
