The Downtown Emergency Service Center (DESC) is a nonprofit entity dedicated to assisting individuals facing the multifaceted challenges of homelessness, substance use disorders, and severe mental health issues. Our mission is to foster a community where every individual is valued, supported, and free from the experience of homelessness.

As a premier provider of services for multiply disabled adults who have encountered chronic homelessness, DESC serves nearly 2,000 individuals daily. Our integrated service model is tailored to help individuals secure and sustain safe, affordable housing. DESC is recognized both nationally and regionally for its innovative approaches to addressing homelessness.

ROLE DESCRIPTION

The Community Outreach & Advocacy Team (COAT) Case Coordinator is an integral member of a multidisciplinary team tasked with delivering outreach, engagement, and intensive case management services to adults. This role employs a modified assertive community treatment (ACT) model, maintaining a manageable caseload of 1:20, ensuring that individuals are identified, assessed, and supported in accessing necessary services and appointments. The team comprises behavioral health professionals, a psychiatric prescriber, and a psychiatric registered nurse, all committed to providing intensive outreach and client-centered care. Responsibilities include all facets of referral, outreach, engagement, and case management, along with coordination with external providers to facilitate successful transitions to ongoing care. Services are delivered throughout the community, including on the streets, in shelters, and at various service centers. Flexibility for evening and weekend hours may be required. This position necessitates a high level of collaboration with other DESC programs and external agencies.

KEY RESPONSIBILITIES:

  • Deliver outreach and engagement services, collaborating with supervisors and team members to connect with new referrals.
  • Conduct a range of case management activities aimed at stabilization and transition to appropriate ongoing providers.
  • Prioritize crisis intervention for clients as necessary, which may involve de-escalation, emergency department visits, and referrals to crisis services.
  • Perform intake evaluations and behavioral health assessments, developing support and treatment plans with maximum client involvement.
  • Oversee all aspects of clients' support and treatment plans, coordinating essential services such as access to food, shelter, clothing, housing stabilization, medical care, psychiatric treatment, financial assistance, and coordination with the criminal justice system.
  • Advocate for clients' access to community resources, ensuring their needs are met and rights upheld; collaborate with community providers to maintain continuity of care.
  • Foster and maintain strong collaborative relationships with DESC staff and other service organizations to ensure comprehensive care for clients.
  • Engage in psychiatric consultations, supervision, program meetings, and in-service training; participate in clinical reviews and case conferences for clients on the caseload.
  • Adhere to the agency's clinical accountability policies and procedures; maintain timely and accurate clinical records; participate in quality assurance reviews as assigned.
  • Comply with relevant program research and evaluation protocols.
  • Perform additional duties as assigned.

LIVING ENVIRONMENT:

Contribute to the planning, organization, and facilitation of unit mitigations for clients on your caseload. Unit mitigations involve coordinated efforts to assist clients in maintaining healthy living conditions, which may include attending care conferences, providing outreach and support in residential settings, coordinating with housing staff for timely maintenance requests, participating in unit cleanouts, and documenting barriers to effective unit management.

QUALIFICATIONS:

  • Relevant Bachelor's degree in social work, psychology, or a related behavioral science field, OR
  • A combination of one year of relevant paid experience and demonstrated capability to fulfill job responsibilities.
  • Genuine interest in working with clients who may be challenging to engage in traditional mental health or substance use programs.
  • Ability to meet state requirements for registration as a Registered Agency Affiliated Counselor (AAC) or any other applicable credential.
  • Effective communication and collaboration skills with diverse staff backgrounds.
  • Ability to work effectively with clients exhibiting a wide range of behaviors.
  • Commitment to a philosophy of cooperation and continuity across programs, with respect and consideration for clients.

PREFERRED QUALIFICATIONS:

  • Eligibility for a Licensed AAC credential or any other credential that meets state requirements to function as a Mental Health Professional, including conducting assessments and making diagnoses.
  • Master's degree in social work, psychology, or a related behavioral science, or a Bachelor of Nursing degree with a mental health specialty.
  • Possession of a valid Washington State driver's license and an insurable driving record for agency-related travel.
  • Bilingual proficiency in Spanish and English.
  • Bicultural experience and/or designation as a Qualified Minority Mental Health Specialist.
  • Experience working with adults who have faced homelessness.
  • Experience supporting adults with mental health and substance use challenges.

PHYSICAL REQUIREMENTS:

The physical demands outlined here represent those that must be met by an employee to successfully perform the essential functions of this role. The employee will also need to bend, stoop, kneel, and reach, as well as lift and/or carry items weighing up to 40 pounds. Proficiency in operating computer hardware systems is also necessary. Specific vision capabilities required for this role include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The role may involve considerable stress.
