DESC (Downtown Emergency Service Center) is a nonprofit entity dedicated to assisting individuals facing the multifaceted challenges of homelessness, substance use disorders, and severe mental health issues. Our mission is to foster a community where no individual is neglected, overlooked, or experiencing homelessness.

As a premier provider of services for multiply disabled adults who have encountered chronic homelessness, DESC supports nearly 2,000 individuals daily. Our comprehensive service model is tailored to help individuals secure and sustain safe, affordable housing. DESC is acknowledged both nationally and regionally for its innovative approaches to addressing homelessness.

ROLE DESCRIPTION:

The COAT (Community Outreach & Advocacy Team) Outreach Worker/Case Coordinator is an integral member of a multidisciplinary team tasked with delivering outreach, engagement, and intensive case management services to adults. This model employs a modified assertive community treatment (ACT) approach with manageable caseloads of 1:20, ensuring individuals are identified, assessed, supported, and accompanied to necessary services or appointments. The team comprises behavioral health professionals, a psychiatric prescriber, and a psychiatric registered nurse, all committed to providing services with an intensive outreach and client-centered focus. This role encompasses all facets of referral, outreach, engagement, and intensive case management, along with coordination with other service providers to ensure a seamless transition for clients to ongoing care. Services will be delivered in various community settings, including streets and shelters, with some evening and weekend hours as required. This position necessitates a high level of coordination and collaboration with other DESC programs and external agencies.

KEY RESPONSIBILITIES:

  • Deliver outreach and engagement services as assigned, collaborating with supervisors and team members when addressing new referrals.
  • Conduct a range of case management activities aimed at stabilization and transition to other ongoing service providers as appropriate.
  • Prioritize crisis intervention for clients as necessary, which may involve de-escalation, emergency department visits, and referrals to crisis systems.
  • Perform behavioral health assessments and create support and treatment plans with maximum client involvement.
  • Take primary responsibility for coordinating all aspects of clients' support and treatment plans, including survival services focused on access to food, shelter, and clothing; rapid acquisition and stabilization of housing; and ongoing medical and psychiatric care.
  • Advocate for clients' access to community resources and services, ensuring their needs are met and rights upheld; consult and collaborate with community providers to guarantee continuity of care.
  • Establish and maintain strong collaborative relationships with DESC staff and other service organizations to ensure comprehensive care for clients.
  • Participate in psychiatric consultations, supervision, program meetings, and in-service training; engage in clinical reviews and case conferences for clients on caseload.
  • Adhere to the agency's clinical accountability policies and procedures; maintain timely and complete clinical records; participate in quality assurance reviews as assigned.
  • Comply with relevant program research and evaluation protocols.
  • Perform other duties as assigned.
LIVING ENVIRONMENT:

Engage in the planning, organization, and facilitation of unit mitigations for clients on your caseload. Unit mitigations involve coordinated efforts to assist clients in maintaining healthy living conditions, which may include attending care conferences, supporting clients in their residences, coordinating with housing staff for timely work orders, and documenting barriers to unit mitigation.

QUALIFICATIONS:

MINIMUM REQUIREMENTS:
  • Relevant Bachelor's degree in social work, psychology, or a related behavioral science field, OR
  • A combination of 1 year of relevant paid work experience and demonstrated ability to perform required job duties.
  • Ability to meet Washington Department of Health requirements for registration as a Registered Agency Affiliated Counselor (AAC) or any other applicable credential.
  • Interest in working with clients who are challenging to engage and retain in traditional mental health/substance use disorder programs.
  • Effective communication and collaboration skills with staff from diverse backgrounds.
  • Ability to work effectively with clients exhibiting a wide range of behaviors.
  • Commitment to the philosophy of cooperation and continuity across programs, with respect and consideration for clients.
PREFERRED QUALIFICATIONS:
  • Eligible for a Licensed AAC credential or any other credential that meets RCW requirements to act as a Mental Health Professional, allowing for independent mental health assessments and diagnoses.
  • Master's degree in social work, psychology, or other relevant behavioral science, or a Bachelor of Nursing degree with a mental health specialty.
  • Ability to drive an agency or personal vehicle for agency-related business, including a valid Washington State driver's license and insurable driving record.
  • Bilingual in Spanish and English.
  • Bicultural background/experience and/or Qualified Minority Mental Health Specialist designation.
  • Experience working with adults who have faced homelessness.
  • Experience working with adults dealing with mental health and substance use challenges.
PHYSICAL REQUIREMENTS:
