Position Responsibilities

The Senior Case Manager supervises the Case Management team, focusing on Black men transitioning from chronic homelessness, many of whom face significant mental or physical health challenges and/or substance use disorders. Benu is devoted to a positive harm reduction approach, fostering a culturally responsive environment that meets individuals where they are, providing non-judgmental and non-coercive services and resources. The Senior Case Manager will collaborate closely with the Case Management team and community residents to assist them in accessing clinical services while working towards permanent housing through the development of personalized goals for enrichment, stability, wellness, and recovery.


Key Duties and Responsibilities:


Clinical Responsibilities

  • Deliver culturally responsive cognitive behavioral therapy (CBT) counseling to residents, both individually and in groups, addressing emotional and mental health concerns.
  • Conduct evaluations and assessments to facilitate referrals to treatment facilities and other community resources tailored to specific resident needs.
  • Establish and maintain strong relationships with clients in both group and individual settings.
  • Provide accurate clinical assessments of residents at Benu Community Home, including mental and emotional health evaluations, as well as general physical assessments in cases of substance abuse.
  • Maintain comprehensive and precise records for each client throughout their treatment journey, whether requested by the client or mandated by a treatment program.
  • Engage in professional consultations with medical professionals, therapists, and legal representatives as necessary.
  • Perform administrative tasks as required to liaise with other social service organizations or government agencies concerning resident needs.
  • Address crises to de-escalate situations involving residents at Benu Community Home.
  • Serve as a site preceptor for students enrolled in social work degree programs when applicable.
  • Other duties as assigned.

People Management

  • Supervise four case managers responsible for resident screening, intake, data collection, program workshops, group facilitation, and weekly meetings with assigned guests.
  • Ensure adequate staffing coverage for a 24-hour enhanced living community.
  • Assist in training and capacity building for case management to stay updated on best practices for culturally responsive care models.
  • Conduct regular meetings with each case manager to review strategic program goals and provide coaching.
  • Maintain accurate records of direct reports' hours worked and ensure proper timesheet submissions.
  • Other duties as assigned.

Supervision of Case Management

  • Guide case managers in identifying specific barriers faced by community residents in achieving their goals and developing necessary life planning skills for appropriate housing.
  • Build rapport, provide support, and advocate for a diverse range of community residents, including those actively using substances or living with significant mental/physical health challenges, employing harm reduction strategies as appropriate.
  • Participate in verbal de-escalation and physical interventions during emergencies, assisting staff in maintaining a safe environment and mediating disputes among residents.
  • Provide training to case managers and advocates on best practices for managing resident behaviors that may hinder their program participation.
  • Offer guidance to case managers on resources for residents to obtain essential housing documentation and, when suitable, delegate a case manager to accompany residents to external appointments.
  • Ensure ongoing engagement with community residents and track progress towards their goals.
  • Facilitate the housing case manager's development of relationships with landlords, referral services, treatment providers, and other community partners to support resident success.
  • Ensure the case management team maintains up-to-date guest records, case notes, and data entry.
  • Other duties as assigned.

Team Collaboration

  • Attend staff meetings, workshops, retreats, community meetings, and in-service training as provided.
  • Contribute to and support a positive, team-oriented work environment; engage in collaborative decision-making with other staff members.
  • Work alongside the Program Coordinator and Director of Programs and Services to ensure program alignment with performance management expectations.
  • Collaborate with the Development and Communication team to identify opportunities for program resources and funding for Benu events and needs.
  • Other duties as assigned.

Working Conditions:

This position requires the employee to work in an environment where exposure to blood, bodily fluids, and other potentially infectious materials may occur. Additional exposures may include cleaning supplies, chemicals used in pest control, paint, and other materials related to building maintenance. Working conditions may involve interruptions, solitary work, interactions with distressed individuals, and exposure to computer screens.


Minimum Qualifications

  • Licensed Clinical Social Worker with a master's degree in social work from an accredited institution.
  • Two or more years of experience in clinical work within a housing program or community-based healthcare setting serving similar demographics.
  • Experience in delivering culturally competent services and effectively engaging with multicultural populations, particularly Black men.
  • Strong understanding of the challenges faced by individuals, including substance abuse, mental health issues, trauma, domestic violence, and chronic homelessness.
  • Knowledge of various therapeutic options and treatments for men, especially Black men experiencing trauma.
  • Demonstrated empathy and emotional intelligence.
  • Commitment to supporting and upholding ACLT's mission, beliefs, and values.
  • Familiarity with local resources that can assist individuals and families.
  • Problem-solving and critical-thinking skills to provide innovative solutions and adaptive strategies for the unique challenges faced by this demographic.
  • Organizational and case management skills and experience.
  • Ability to collaborate and work effectively as part of a team.
  • Proficient computer skills and familiarity with Office 365, Google Docs, databases, and electronic medical records.
  • Willingness to learn and work within a diverse environment.
  • Familiarity with best practices for addressing the unique experiences of African American men.
  • Commitment to a harm reduction program model.
  • Experience in navigating ambiguity in a fast-paced environment.
  • Experience with data collection, presentation, and report writing.

Preferred Qualifications:

  • Knowledge of the local subsidized housing and low-rent private housing market.
  • Experience working with chronically homeless adults.
  • Proficiency in a second language.
  • CPR / First Aid certification.
