Overview:
The Evergreen Treatment Services REACH Program is dedicated to assisting individuals facing homelessness and mental health challenges in achieving stability and enhancing their quality of life. Our outreach initiatives emphasize relationship-building, advocacy, and collaboration to minimize harm and foster recovery. We offer comprehensive outreach-based care coordination, multidisciplinary clinical services, and support for accessing and maintaining housing. Our approach is rooted in harm reduction principles, ensuring respect and dignity for individuals navigating their personal journeys of change.

Services Offered:
We provide a wide range of services, from survival support for those living outdoors to essential resource linkages, including housing assistance, legal support, healthcare access, and treatment for substance use and mental health conditions.

Key Responsibilities:
• Engage with clients in designated outreach areas to conduct health and needs assessments.
• Establish and maintain positive relationships with community partners and referral sources.
• Facilitate access to services such as shelter, case management, and healthcare.
• Document outreach activities and maintain accurate client data.
• Collaborate with city and service partners to develop effective outreach strategies.
• Participate in case conferencing and community meetings to enhance service delivery.

Qualifications:
Education:
• High school diploma or equivalent required; a degree in social services is preferred.

Experience:
• Minimum of two years in social work or outreach-related roles.
• Proven ability to engage respectfully with individuals experiencing homelessness and behavioral health issues.

Skills:
• Proficient in Microsoft Office and capable of managing multiple tasks efficiently.
• Strong interpersonal skills with the ability to work collaboratively across diverse communities.

Physical Requirements:
• Ability to perform tasks that may require lifting and mobility within various environments.
