Client Services Coordinator

Position Title: Case Manager
Reports to: Director of Social Services
FLSA Status: Non-Exempt (Hourly) / Full Time
POSITION SUMMARY:
The Case Manager is responsible for delivering direct case management services to clients and residents.
Key Responsibilities:
- Schedule initial meetings with newly assigned clients to evaluate their fundamental needs and create a tailored service plan.
- Conduct comprehensive psychosocial assessments to identify the unique requirements of each client.
- Manage an assigned caseload of clients and residents effectively.
- Fulfill all essential casework obligations and make necessary referrals.
- Complete all required documentation, including case management notes and referral sheets, promptly.
- Address and resolve client incident reports and complaints as necessary.
- Participate in mandated consumer hearings and discharge processes.
- Monitor client progress, prepare periodic reports for each case, and assess the effectiveness of service plans.
- Develop and implement individualized treatment plans, tracking progress and evaluating outcomes.
- Make appropriate referrals to resources that meet the specific needs of residents, determining eligibility for services.
- Engage in weekly supervision meetings for case and administrative guidance.
- Respond to emergencies as they occur, coordinating medical care or other necessary services.
- Attend professional development seminars and staff meetings as required.
Qualifications:
To excel in this role, candidates must meet the following qualifications:
- Education & Experience:
- Bachelor's Degree in Social Work or a related discipline.
- Certification in First Aid and CPR.
- Preferred Experience:
- At least 2 years of relevant experience.
- Experience working with homeless populations, MICA, or the Veterans community is advantageous.
- Technical Skills:
- Proficient in various computer software applications (e.g., Word, Excel, PowerPoint, CARES).
- Communication Skills:
- Exceptional verbal and written communication abilities. Capable of reading and understanding simple instructions, writing basic correspondence, and presenting information effectively in small group settings.
- Bilingual proficiency is preferred.
