Position Summary:

We are looking for a motivated and enthusiastic Care Coordinator to become part of our dedicated team. This full-time role will focus on managing a diverse caseload of clients within our Health Home Care Management initiative.

Key Responsibilities:

The Care Coordinator will:

  • Oversee a caseload of over 85 Health Home Care clients.
  • Assist in formulating Comprehensive Care Plans tailored to individual needs.
  • Address a variety of service requirements, including housing, benefits, medical care, transportation, education, employment, and crisis intervention.
  • Collaborate with a multidisciplinary Care Team, including supervisors, clinicians, psychotherapists, and psychiatrists.
  • Conduct advocacy, assessments, service planning, and maintain accurate documentation in Electronic Health Records.
  • Engage with clients diagnosed with mental health issues, substance use disorders, and chronic medical conditions on a regular basis.

Qualifications:

The ideal candidate will possess:

  • Bilingual proficiency in English and Spanish is required.
  • A Bachelor's Degree in a relevant field.
  • Experience with RMA is essential.
  • Familiarity with HARP clients is preferred.
  • Knowledge of community resources and services to enhance client support.
  • Strong verbal and written communication skills to deliver culturally competent services.
  • The ability to collaborate with other professionals and organizations regarding client needs.

Work Environment:

Salary Range: $45,000 - $50,000, commensurate with experience and qualifications.
