Client Services Representative

**About the Role**
Ultimate Care NY is seeking a skilled Care Coordination Specialist to join our team. As a key member of our team, you will play a vital role in ensuring a seamless intake process for clients seeking home care services.
Key Responsibilities:
- Collaborate with internal teams to facilitate a smooth intake process for clients.
- Conduct thorough intake assessments to determine client needs and eligibility for services.
- Educate and inform consumers about the range of care services available.
- Guide clients through the enrollment process, ensuring all necessary paperwork and requirements are met.
- Maintain accurate and up-to-date records of client information and interactions.
- Provide exceptional customer service by addressing inquiries and concerns promptly and professionally.
- Exhibit a positive attitude and strong teamwork skills, fostering a supportive and collaborative work environment.
Requirements:
- Previous experience in home care intake coordination is preferred, but we are willing to train the right candidate who exhibits enthusiasm and a willingness to learn.
- Customer service experience in a home care environment is a plus, showcasing your ability to empathize and communicate effectively with clients and their families.
- Excellent organizational skills, attention to detail, and the ability to prioritize tasks to ensure efficient workflow.
- A proactive go-getter attitude that enables you to find creative solutions and make the most of available resources.
- A positive attitude: you are a team player, and you work effectively with different teams.
- Proficiency in Microsoft Office Suite.
