Job Overview

SHIFT: Monday-Friday 8am-4:30pm

POSITION SUMMARY:

This role is dedicated to Interfaith's California Advancing and Innovating MediCal (CalAIM) Recuperative Care Program, which focuses on medical respite care. The Case Manager will deliver intensive case management services to individuals facing significant medical, psychiatric, and substance use recovery challenges, particularly those experiencing homelessness or unstable living conditions. These individuals may be too ill or frail to recuperate from an illness or injury in their typical living environment, yet not sick enough to require hospitalization. The medical respite setting facilitates the achievement of medical stability, prevents hospital admissions or readmissions, and promotes improved psychosocial functioning.

KEY RESPONSIBILITIES:

To excel in this position, the individual must competently perform each essential duty. The following responsibilities are considered vital but do not encompass all functions that may be required:

  • Deliver comprehensive case management to individuals lacking stable housing and with high medical or behavioral health needs.
  • Conduct intake assessments, including bio-psychosocial evaluations and housing assessments.
  • Design and implement collaborative care management and housing support plans tailored to individuals' needs across physical health, mental health, substance use disorders, community services, and social determinants of health.
  • Assist clients in accessing benefits and necessary documentation, such as Social Security Insurance (SSI), CalFresh, and other relevant records.
  • Facilitate discharge planning to suitable community housing, including application assistance and resource linkage.
  • Effectively collaborate and communicate with an interdisciplinary care team and external partners.
  • Coordinate care with managed care plans, community providers, and authorized support persons.
  • Provide crisis intervention as part of the care team.
  • Engage in individual and group case consultations.
  • Complete clinical and administrative reports as assigned, ensuring timely submission.
  • Maintain accurate and up-to-date documentation for services rendered.
  • Adapt schedule flexibly according to client needs.
  • Participate in staff meetings and training sessions.
  • Perform additional duties as assigned.

Case managers may work with both Veteran and non-Veteran populations.

EXPERIENCE / EDUCATION:

  • Master's Degree in Social Work (MSW), Marriage and Family Therapy (MFT), or a related field is required.
  • License-eligible and currently registered with the State of California Board of Behavioral Sciences as an Associate Social Worker (ASW), Associate Marriage and Family Therapist (AMFT), or related field is preferred.
  • Two (2) or more years of relevant work experience is required.
  • Knowledge of substance use, mental health assessments, intervention strategies, and coordinated care in a community non-profit setting is preferred.
