Position Overview:

Key Duties:

  • Offer a variety of client support services such as outreach, assessment, crisis intervention, advocacy, and case coordination.
  • Conduct thorough intake evaluations for all incoming clients.
  • Maintain accurate documentation of client interactions through case notes, treatment plans, and progress reports, ensuring compliance with contractual requirements.
  • Guide clients in identifying their strengths, establishing achievable goals, and providing ongoing support to promote self-sufficiency.
  • Implement supportive services in accordance with organizational policies and contractual obligations.
  • Connect clients with appropriate resources within the organization and through external public and private social service entities.
  • Deliver services in diverse community environments, including other organizational programs, faith-based centers, and educational institutions.
  • Adhere to departmental and organizational policies and procedures.
  • Organize and facilitate monthly resident meetings, social gatherings, and educational classes (such as Life Skills and Active Recovery) to engage clients in alignment with grant and contract stipulations.
  • Assist the Program Manager in gathering data for program evaluations.
  • Actively participate in staff meetings and training sessions.
  • Utilize Housing First principles, Trauma-Informed Care, and Harm Reduction strategies in client interactions.
  • Perform additional responsibilities as needed.

Schedule: Monday through Friday, 8 AM to 5 PM

Language Preference: Spanish Preferred

Qualifications:

- Background in Social Work or Case Management

- Knowledge of the social security process

- Familiarity with populations experiencing substance abuse, co-occurring disorders, or homelessness

- Bachelor’s degree in social services required or equivalent experience in lieu of degree
