Job Summary

We are seeking a highly skilled and compassionate Bilingual Therapist to join our team as an Intensive Care Coordinator. As a key member of our multidisciplinary team, you will work closely with families, youth, and community partners to provide comprehensive support services.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized treatment plans to address the unique needs of each family.
  • Provide intensive case management services, including crisis intervention, collateral contacts, and coordination of community resources.
  • Collaborate with social workers, probation officers, schools, and community partners to ensure a cohesive and effective support system.
  • Develop and maintain relationships with community resources, including informal and formal networks, to provide linguistically and culturally diverse support services.
  • Participate in fidelity and quality improvement efforts, including wrap coaching, ongoing training, and supervision.
  • Maintain accurate and timely case records, adhering to Medi-Cal requirements and SDCC policies.
Requirements
  • Master's degree from an accredited university in a relevant field (e.g., MSW, MFT, APCC).
  • Valid California driver's license and reliable transportation.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Bilingual Spanish language proficiency.
  • Experience working with SED youth and families, preferably in a Wraparound program.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and communication.
Preferred Qualifications
  • Experience documenting in an Electronic Health Record (EHR) system.
  • Proficiency in Cerner (online medical records system) and MediCal billing requirements.
  • Experience working in a Wraparound program, with a focus on family-centered care and community-based services.
