CalAIM Care Coordinator

WORK SCHEDULE: Monday-Friday 8am-4:30pm
POSITION SUMMARY:
This role is dedicated to Interfaith's California Advancing and Innovating MediCal (CalAIM) Recuperative Care Program, focusing on medical respite care. The Case Manager will deliver comprehensive case management services to individuals facing significant medical, psychiatric, and substance use recovery challenges, particularly those experiencing homelessness or unstable living conditions. This position is crucial for individuals who are too ill or frail to recuperate in their usual environment but do not require hospitalization. The medical respite setting aims to support individuals in achieving medical stability, preventing hospital admissions or readmissions, and enhancing their psychosocial functioning.
KEY RESPONSIBILITIES:
To excel in this role, the individual must effectively fulfill the essential duties outlined below. Reasonable accommodations may be provided to enable individuals with disabilities to perform these functions:
- Deliver thorough case management to individuals lacking stable housing and experiencing high medical or behavioral health needs.
- Conduct intake assessments, including bio-psychosocial evaluations and housing assessments.
- Develop and execute collaborative care management and housing support plans tailored to individuals' needs across physical health, mental health, substance use disorders, community services, and social determinants of health.
- Assist clients in accessing additional benefits and necessary documentation, such as Social Security Insurance (SSI), CalFresh, and other essential records.
- Facilitate discharge planning to suitable community housing, including application assistance and resource linkage.
- Engage in effective collaboration and communication with interdisciplinary teams and external care providers.
- Coordinate care with managed care plans, community providers, and authorized support persons.
- Provide crisis intervention as part of the care team.
- Participate in individual and group case consultations.
- Complete clinical and administrative reports as required by Interfaith and CalAIM within designated timeframes.
- Maintain accurate and up-to-date documentation for services rendered to clients.
- Adapt work schedules flexibly according to client needs.
- Engage in all staff meetings and training sessions.
- Perform additional duties as assigned.
Case managers may be assigned to work with either Veteran or non-Veteran populations.

QUALIFICATIONS:
- Master's Degree in Social Work (MSW), Marriage and Family Therapy (MFT), or a related field is required.
- License-eligible and currently registered with the State of California Board of Behavioral Sciences as an Associate Social Worker (ASW), Associate Marriage and Family Therapist (AMFT), or related field is preferred.
- A minimum of two (2) years of relevant work experience is required.
- Familiarity with substance use, mental health assessments, intervention strategies, and coordinated care in a community non-profit setting is preferred.
