Interfaith Community Services is seeking a skilled Case Manager to join our team in the California Advancing and Innovating MediCal (CalAIM) Recuperative Care Program. As a Case Manager, you will provide intensive case management to individuals experiencing high medical, psychiatric, and substance use disorder recovery needs, with a focus on medical respite care.
Key Responsibilities- Conduct comprehensive bio-psychosocial needs assessments and develop collaborative care management plans to support individuals' physical health, mental health, and social determinants of health.
- Assist individuals in accessing benefits, such as Social Security Insurance (SSI), CalFresh, and cash aid, and provide documentation support.
- Coordinate care with managed care plans, community providers, and external care teams to ensure seamless transitions and independence.
- Provide crisis intervention and participate in individual and group case consultation to ensure effective case management.
- Maintain up-to-date documentation for services provided and adhere to allocated time frames.
- Master's Degree in Social Work (MSW), Master's Degree with an emphasis in Marriage and Family Therapy (MFT), or related field preferred.
- Two (2) or more years of relevant work experience required, with knowledge of substance use, mental health, assessments, intervention, and coordinated care strategies in a community non-profit setting.
