CalAIM Care Coordinator

ROLE SUMMARY:
This role is dedicated to Interfaith's California Advancing and Innovating MediCal (CalAIM) Recuperative Care Program, focusing on medical respite care. The Case Manager is tasked with delivering intensive case management services to individuals facing significant medical, psychiatric, and substance use recovery challenges, particularly those experiencing homelessness or unstable living conditions. This position supports individuals who are too ill or frail to recuperate in their usual living environments but do not require hospitalization. The medical respite setting facilitates the achievement of medical stability, aids in the prevention of hospital admissions or readmissions, and promotes improved psychosocial functioning.
KEY RESPONSIBILITIES:
To excel in this position, the individual must effectively manage the following responsibilities:
- Deliver comprehensive case management for individuals lacking stable housing and facing high medical or behavioral health needs.
- Conduct thorough intake and bio-psychosocial assessments, including housing evaluations.
- Design and implement collaborative care management and housing support strategies tailored to the individual's physical health, mental health, substance use issues, community services, and social determinants of health.
- Assist clients in navigating additional benefits and necessary documentation, such as Social Security Insurance (SSI), CalFresh, and other essential records.
- Facilitate discharge planning to suitable community housing, including application support and resource linkage to foster independence and stable housing transitions.
- Engage in effective collaboration and communication with an interdisciplinary care team and external partners.
- Coordinate care with managed care plans, community providers, and authorized support persons.
- Provide crisis intervention as part of the team effort.
- Participate in individual and group case consultations.
- Complete assigned clinical and administrative reports within designated timeframes.
- Maintain accurate and up-to-date documentation of services provided.
- Adapt work schedules to meet client needs.
- Engage in all staff meetings and training sessions.
- Perform additional duties as assigned.
Case managers may be designated to work with either Veteran or non-Veteran populations.

EXPERIENCE / EDUCATION REQUIREMENTS:
- Master's Degree in Social Work (MSW), Marriage and Family Therapy (MFT), or a related field is required.
- License-eligible and currently registered with the State of California Board of Behavioral Sciences as an Associate Social Worker (ASW), Associate Marriage and Family Therapist (AMFT), or in a related field is preferred.
- A minimum of two (2) years of relevant work experience is required.
- Familiarity with substance use, mental health assessments, intervention strategies, and coordinated care in a community non-profit setting is preferred.
