About the Role

Evergreen Treatment Services is seeking a highly motivated and compassionate Care Coordinator to join our REACH Program team. As a Care Coordinator, you will play a critical role in providing outreach, engagement, and intensive case management services to Spanish-speaking individuals experiencing homelessness and substance use disorders.

Key Responsibilities
  • Outreach and Engagement: Develop and maintain relationships with clients, providing culturally sensitive and individualized support to help them achieve stability and improved quality of life.
  • Case Management: Assess clients' needs and develop comprehensive service plans to address their physical, emotional, and social well-being.
  • Advocacy: Collaborate with community programs and service providers to ensure clients receive necessary resources and support.
  • Documentation: Maintain accurate and up-to-date client records, adhering to agency and contract requirements.
  • Communication: Effectively communicate with clients, staff, and external partners to ensure seamless service delivery.
Requirements
  • Education: High school diploma or equivalent required; undergraduate degree preferred.
  • Experience: Minimum two years' experience in social work or outreach programs, with a focus on substance use disorder treatment and harm reduction.
  • Language Skills: Bilingual in Spanish and English, with the ability to communicate effectively with clients and staff.
  • Knowledge and Skills: Strong understanding of substance use disorders, homelessness, and co-occurring behavioral health conditions; ability to work independently and as part of a team.
