Job Summary

We are seeking a skilled Case Manager to join our team at the Low Income Housing Institute. As a Case Manager, you will play a critical role in assisting formerly homeless individuals to stabilize in permanent supportive housing.

Key Responsibilities
  • Information and Referral/Case Management
    • Build effective relationships with clients to provide personalized support and guidance.
    • Assess tenants' current level of functioning and social service needs to develop tailored case management plans.
    • Offer case management services to selected clients, either referred by building management or self-identified.
    • Make referrals or initiate services to address mental health, substance abuse, and disability issues.
    • Coordinate with local agencies to provide services that meet the needs of LIHI clients.
  • Community Building
    • Coordinate with community organizations, public agencies, and volunteers to provide on-and off-site activities for clients.
    • Initiate and encourage client involvement in community-building activities, such as community meetings, meals, and support groups.
    • Produce written materials, including community newsletters, to promote community engagement.
  • Administrative
    • Participate in staff meetings and planning sessions to contribute to the development of community programs.
    • Report regularly to Supportive Services Supervisor on programmatic issues.
    • Maintain records of activities and contacts with tenants/agencies and case management records.
  • Other
    • Participate in public policy advocacy efforts to promote affordable housing and social services.
    • Assist with fundraising efforts to support community programs.
    Requirements
    • Bachelor's Degree in Social Work or related field, or Associate degree with 3 years of experience.
    • At least two years' experience working with a diverse population.
    • CPR/First Aid certification desired.
    • Background in case management, resident advocacy, or related career required.
    • Knowledge of referral resources and experience working with confidential information.
    • Excellent written and communication skills.
    • Access to transportation.
    • Flexibility to work evenings or weekends as needed.
    • Ability to pass criminal background checks and drug screening test.
