Position Overview:
Under the supervision of the Director of Gender-Related Services, the GRS Case Manager is responsible for providing comprehensive support and counseling to patients within correctional facilities. This role includes the development and implementation of tailored discharge plans, as well as follow-up support after release.

Key Qualifications:
1. Valid counseling license (LMSW, LMHC, LMFT, etc.)
2. Experience in providing services to individuals affected by Intimate Partner Violence
3. A minimum of two years of experience in the correctional system is highly preferred; experience in the court system or related criminal justice fields will also be considered.
4. A strong commitment to supporting vulnerable populations
5. An established interest in women's issues, LGBTQI+ advocacy, health policy, compassionate release, and incarceration matters.

Proficiency in using Excel, electronic medical records, and other relevant software is essential.

Responsibilities:

Conduct intake assessments, develop safety plans, and create discharge plans for patients assigned to your caseload, which includes diverse populations served by GRS. Meet with patients across various facilities as necessary, in consultation with management and medical staff. Facilitate counseling sessions focused on IPV for patients experiencing current concerns. Assist patients in applying for Medicaid and other benefit programs as needed.

Design individualized discharge plans that align with each patient's goals and needs, ensuring that all necessary tasks are completed.

Additional Duties:

These may include coordinating medical appointments, ensuring prescriptions are filled at pharmacies, making referrals to housing and case management resources, and arranging transportation between correctional facilities and community residences.

Provide consistent in-jail support to patients, meeting with them regularly, with frequency determined in collaboration with the Program Director.

Act as a liaison between patients and clinical court advocates, assisting with obtaining consents, relaying information, and performing other necessary tasks.

Participate in weekly team meetings and contribute to patient case discussions with other staff involved in discharge planning.
Conduct follow-up outreach to patients post-release via phone, providing assistance or referrals as appropriate.
Document all assessments, transition plans, progress notes, and relevant information in the electronic health record system (CHER).

Additional projects and tasks may be assigned by the Program Manager or Director of Special Populations, which may include research, material development, and training sessions.

Minimum Requirements:
1. A high school diploma or its equivalent, along with four years of relevant full-time experience in direct counseling, crisis intervention, or referral services for inmates or similar populations; or
2. An equivalent combination of education and experience, where thirty semester credits from an accredited institution are considered equivalent to one year of required experience. However, all candidates must possess a high school diploma or its equivalent and at least one year of the described experience.
