Job Summary

We are seeking a highly motivated and compassionate Client Care Coordinator to join our team at The Landing Family Shelter. As a key member of our social services team, you will be responsible for providing critical support to families in crisis, helping them to achieve stability and self-sufficiency.

Key Responsibilities

  • Assist clients in completing intake applications, screenings, and forms, ensuring accurate and timely documentation.
  • Conduct initial risk assessments of clients and their families, identifying areas of need and developing effective service plans.
  • Prepare and update biopsychosocial evaluations, ensuring compliance with regulatory requirements.
  • Provide instant response services to clients in crisis, ensuring their immediate well-being and safety.
  • Collaborate with clients and case managers to develop and implement Independent Living Plans (ILPs), setting short-term and long-term goals.
  • Identify and connect clients with community resources, including medical, psychiatric, and educational services.
  • Follow up with clients and referral organizations to ensure progress and outcomes.
  • Consult with internal and external stakeholders to determine causes of client problems and develop effective solutions.
  • Act as an advocate for clients, arranging for necessary tests and examinations to identify underlying issues.
  • Facilitate case conferences with stakeholders to ensure a collaborative approach to services.
  • Provide regular progress updates and documentation, ensuring accurate and timely reporting.
  • Lead group counseling sessions and psychoeducation programs to enhance social development and psychological education.
  • Monitor client progress, reassessing service plans and goals as needed.
  • Provide aftercare services to ensure client stability and continued support.

Requirements

  • Licensed Master Social Worker (LMSW) required within one year of hire, with a commitment to maintaining licensure throughout employment.
  • State Central Registry (SCR) clearance, Staff Exclusion List (SEL) clearance, and Criminal history information checks via fingerprinting with New York State Division of Criminal Justice Services prior to start of employment.
  • Ability to maintain clearances throughout the duration of employment.
