Community Connections Service Case Manager

Position Overview:
This full-time role is integral to the Community Connections Service (CCS) team, operating within the Reentry and Transition Services Department.
The CCS team comprises jail-based professionals dedicated to assisting individuals recently incarcerated, focusing on benefits applications such as Medicaid and securing consents for legal and community support.
This position is situated in a correctional facility and involves direct service responsibilities.
Key competencies required include exceptional communication abilities, foundational computer skills, resourcefulness, organizational capabilities, comfort in navigating correctional environments, and adaptability. Ideal candidates will exhibit a friendly demeanor and possess experience in correctional settings.
Key Responsibilities:
- Welcome individuals newly admitted to the correctional system.
- Facilitate the acquisition of voluntary patient consents (e.g., HIPAA).
- Digitally scan and upload documentation to the patient's electronic health record.
- Navigate through jail-based clinics and/or housing areas to deliver services.
- Undertake additional tasks and projects as directed by leadership.
Preferred Qualifications:
- Ability to independently manage tasks.
- Strong interpersonal and communication skills.
- Basic proficiency in computer usage.
- Comfort in engaging with individuals in a correctional context.
- Familiarity with the criminal justice and social service systems.
- Understanding of benefit entitlements, especially Medicaid.
Minimum Requirements:
- A high school diploma or its equivalent, along with four (4) years of relevant full-time experience providing direct counseling, guidance, crisis intervention, or referral services for inmates or similar populations; or
- Education and experience equivalent to the above. A total of thirty (30) semester credits from an accredited institution will be considered equivalent to one (1) year of the required experience.
All candidates must possess a high school diploma or its equivalent and at least one (1) year of experience as outlined above.
