Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at Women In Need, Inc. as a Transitional Housing Specialist. The successful candidate will be responsible for providing case management services to clients in our transitional housing program, ensuring their successful integration into housing and employment goals.
Key Responsibilities- Initial Assessment and Planning
Conduct a thorough initial assessment of each client within 48 hours of arrival, including completion of the Family Case Assessment Form, Initial Service Plan, and all necessary intake documentation.
Referral and Intake ServicesInitiate referrals for emergency services as needed and complete intake information in the Uniform Client Chart, ensuring accurate and timely data entry.
Client Orientation and SupportProvide comprehensive orientation to clients, reviewing facility rules, client rights and responsibilities, grievance procedures, and emergency protocols.
Case Management and SupportOffer crisis intervention, counseling services, and other support services to clients, utilizing Evidence-Based Practices (EBP) such as strength-based case management, motivational interviewing, and trauma-informed care.
Independent Living Plan DevelopmentDevelop and implement Independent Living Plans (ILPs) for each client, defining goals and tracking progress, making modifications as needed to ensure compliance.
Training and CoachingProvide hands-on training, coaching, and support to clients, helping them develop skills to achieve their ILP goals.
Client Data ManagementMaintain accurate and up-to-date client data in the Uniform Client Chart.
RequirementsThe ideal candidate will possess a strong understanding of case management principles, excellent communication and interpersonal skills, and the ability to work effectively with diverse populations. A bachelor's degree in a related field and relevant experience in case management or a related field are required.
