We are currently seeking an enthusiastic Community Outreach Coordinator to join our North County team. Ideal candidates will have a background in hospice or healthcare marketing, as well as experience in senior care management. A minimum of two years in healthcare-related marketing, care management, or educational roles is preferred. Bilingual proficiency in English and Spanish is an added advantage. This position will primarily engage with the North Inland San Diego area.

Job Title: Community Outreach Coordinator for Hospice Services

Reports to: Director of Partner Relations

Qualifications:

  • Education: A college degree in gerontology, nursing, a health-related field, business, education, or marketing is preferred, or three years of experience in senior care living, such as RCFE administration or resident service care management.
  • Experience: At least three years of experience in marketing, care management, or senior care within the healthcare sector, including hospice, home health, or related fields, is preferred.
  • Other: Must possess a valid driver's license, reliable transportation, and auto insurance.

This role is essential in effectively promoting and educating the community about HNC's palliative and hospice services. Key competencies include:

  • Organizational Skills: Highly organized and detail-oriented, ensuring efficient task execution and goal achievement.

Summary of Major Responsibilities:

  • Conduct outside marketing outreach to potential and existing referral sources on behalf of Hospice of the North Coast, meeting clearly defined benchmarks such as referral growth and implementing strategic plans.
  • Promote and position HNC's programs and services to assisted living communities, skilled nursing facilities, medical professionals, and the community through regular visits, reflecting HNC's commitment to quality care.
  • Build and maintain relationships with external and internal stakeholders to foster a collaborative environment.
  • Provide ongoing support to assisted living and skilled nursing facilities while assisting with resident care coordination.
  • In this purpose-driven role, you will introduce and market our Hospice & Palliative Care services to industry professionals and the community, illustrating the quality of HNC's care philosophy.
