We are currently seeking a passionate Community Outreach Coordinator to enhance our North County operations. The ideal candidate will have a background in hospice or healthcare marketing, along with experience in senior care management. A minimum of two years in healthcare-related marketing or care management is preferred. Bilingual skills in English and Spanish are advantageous. This position will primarily serve the North Inland San Diego area.

Position Title: Community Outreach Coordinator for Hospice Services

Reporting To: Director of Partner Relations

Qualifications:

  • Education: A college degree in gerontology, nursing, a health-related field, business, education, or marketing is preferred, or three years of experience in senior care living, such as RCFE administration or resident service care management.
  • Experience: A minimum of three years in marketing, care management, or senior care within the healthcare sector, including hospice, home health, or related fields, is preferred.
  • Other: A valid driver's license, reliable transportation, and auto insurance are required.

This role is essential in effectively promoting and educating the community about our hospice and palliative care services. Key competencies include:

Key Responsibilities:

  • Conduct outreach marketing calls to potential and existing referral sources on behalf of Hospice of the North Coast, achieving defined benchmarks such as referral growth.
  • Promote and position our programs and services to assisted living communities, skilled nursing facilities, and medical professionals through regular visits.
  • Build and maintain relationships with external and internal stakeholders to foster collaboration.
  • Provide ongoing support to assisted living and skilled nursing facilities while coordinating resident care.
  • Introduce and market our Hospice & Palliative Care services to industry professionals and the community, reflecting our commitment to quality care.
