Job Title: Community Outreach Coordinator for Hospice Services

Reports To: Director of Partner Relations

Qualifications:

  • Education: A college degree in gerontology, nursing, a health-related field, business, education, or marketing is preferred, or three years of experience in senior care living.
  • Experience: At least three years of experience in marketing, care management, or senior care within the healthcare sector is preferred.
  • Other: A valid driver's license, reliable transportation, and auto insurance are required.

Key Responsibilities:

  • Conduct outreach calls to potential and existing referral sources, meeting defined benchmarks for referral growth.
  • Promote and position our programs and services to assisted living communities, skilled nursing facilities, and medical professionals.
  • Build and maintain relationships to foster a collaborative environment.
  • Provide ongoing support to assisted living and skilled nursing facilities in coordinating resident care.
  • Market our Hospice & Palliative Care services to industry professionals and the community, reflecting our commitment to quality care.
