Position Summary:

The Activities Facilitator will support the Activities Coordinator in the Memory Care unit by assisting in the creation and implementation of a high-quality program of activities tailored to the interests and well-being of residents. This program aims to enhance the physical, mental, and emotional health of each individual in the Memory Care neighborhoods.

Key Responsibilities:

  1. Assist in planning, developing, and executing activities that enrich the lives of Memory Care residents, including the preparation of monthly activity calendars.
  2. Lead and facilitate one-on-one and group activities of varying sizes, ensuring all necessary preparations and clean-up are completed. Encourage participation from residents and their families, and provide transportation as needed.
  3. Maintain accurate documentation such as attendance records, activity history, and transportation logs.
  4. Foster effective working relationships within the Memory Care department and collaborate with other departments to meet resident needs.
  5. Support media communication efforts, including newsletters and bulletin boards, and assist in the preparation of seasonal decorations.
  6. Work with the Activities Coordinator and Memory Care Manager to evaluate the effectiveness of activity programs and assess individual resident needs.
  7. Participate in resident and family council meetings, advocating for residents' rights.
  8. Engage in training and professional development activities as required.

Required Skills and Qualifications:

  1. Excellent communication skills with individuals of all ages, combined with a positive and energetic demeanor.
  2. Ability to work effectively in a team-oriented environment.
  3. Strong organizational and time management skills.
  4. Proficient in computer applications including Word, Excel, Outlook, and PowerPoint.
  5. Ability to interpret and analyze professional documents and regulations.
  6. Competence in writing reports and correspondence.

Additional Requirements:

  1. Proficiency in reading and writing English, with the ability to follow verbal and written instructions.
  2. Experience working with the elderly and those with dementia is preferred.
  3. Flexibility to work varied hours, including weekends and evenings.
  4. A high school diploma is required; higher education is preferred.
  5. Preferred certification as a Therapeutic Recreation Specialist or Activities Professional from a recognized body.
  6. A valid driver’s license is required.

Mental and Physical Demands:

  1. Ability to stand and walk for extended periods.
  2. Capable of sitting, using hands to handle objects, and reaching overhead.
  3. Regularly required to communicate verbally and hear; specific vision abilities required include close vision and the ability to adjust focus.
  4. Must be able to lift and/or move up to 25 pounds and push heavier weights as necessary.
  5. Ability to assist in emergency evacuations.
  6. Maintain composure and positivity in stressful situations.
