Role Summary

The Patient Referral Coordinator plays a vital role in managing patient referrals, ensuring seamless communication and coordination among healthcare providers. This position is essential for facilitating the transition of patients from various care settings, including rehabilitation and chronic disease management.

Our team is composed of highly skilled professionals who work collaboratively within an interdisciplinary framework to deliver specialized care tailored to each patient's unique needs, ultimately aiming to enhance their independence and quality of life.

Key Responsibilities
  • Process and manage both internal and external patient referrals by collecting relevant clinical and non-clinical information.
  • Input data into our computerized systems accurately and efficiently.
  • Communicate pertinent information to all stakeholders involved in patient care.
  • Address inquiries from both external and internal sources, providing necessary information and assistance.
Qualifications and Skills
  • 2-3 years of experience in a clerical or administrative role.
  • Proficiency in Microsoft Office and a willingness to learn new software applications.
  • Flexibility to adapt to a varied and changing work schedule.
  • Familiarity with basic medical terminology is preferred.
  • Understanding of insurance processes, managed care, and discharge planning in home health is advantageous.
  • Exceptional communication and customer service skills are essential.
  • Strong written, verbal, and electronic communication abilities.
  • Capability to interact effectively with a diverse patient population.
  • Excellent organizational skills with a keen attention to detail.
