Transition Coordinator

Position Overview: The primary objective of this role is to engage an individual who can offer guidance, support, and assistance in facilitating the transition from educational settings to adult life for students aged 11 to 18 with special educational requirements.

Management of the Learning Environment:

  • Collaborate with district staff on all facets of transition planning and programming as students with special educational needs progress from high school to community and postsecondary education and/or employment.
  • Disseminate information regarding transition and provide professional development opportunities for educators and administrators.
  • Support families, parents, and students in accessing resources and understanding transition services.
  • Act as a liaison between vocational programs and special education instructors to ensure effective student support.
  • Work with guidance counselors to formulate post-secondary pathways for students.
  • Assist vocational educators in modifying curriculum to meet diverse student needs.
  • Identify, establish, and maintain connections with adult service agencies, including making necessary referrals.
  • Participate in Individualized Education Program (IEP) meetings.
  • Draft transition plans and develop suitable transition objectives.
  • Design and implement a transition planning process.
  • Create and execute a transition curriculum that promotes the achievement of transition goals.
Supervision:

Scope of Supervision:

Oversee students with moderate to severe special educational needs, assessing, supporting, and encouraging successful transitions.

Supervision Structure:

Reports to the Administrator of Pupil Personnel Services or the Assistant Director of Special Education, Secondary.

Qualifications: Qualified candidates are encouraged to submit a cover letter, resume, academic transcripts, proof of licensure, and three current letters of recommendation.

Education, Training, and Experience:
  • Bachelor's or Master's Degree from an accredited institution.
  • Initial or Professional license as a Teacher of Students with Moderate Disabilities, Teacher of Students with Severe Disabilities, Teacher of the Visually Impaired, Teacher of the Deaf and Hard of Hearing, School Guidance Counselor, or School Social Worker/Adjustment Counselor.
  • MA DESE Transition Specialist Endorsement or equivalent.
  • Demonstrated expertise in the area of licensure.
Knowledge Required:
  • Foundations and execution of transition education and services.
  • Individual transition assessments and system evaluations.
  • Development of transition systems and supports.
  • Understanding of Special Education laws and regulations.
  • Knowledge of transition planning for both moderate and severe student populations.
Abilities Required:
  • Ability to plan, develop, implement, evaluate, and promote culturally responsive transition services for youth with diverse abilities and disabilities, including those in out-of-district placements.
  • Ability to conduct, manage, interpret, and report on transition assessment activities, including assistive technology where applicable, across various domains such as self-determination, academics, vocational skills, independent living, and community engagement.
  • Ability to create work-based learning plans and individualized learning plans based on appropriate assessments.
  • Ability to connect students with adult services, community providers, and vocational rehabilitation services as part of their life and career planning.
Skills Required:
  • Effective listening, speaking, reading, and writing abilities.
  • Strong interpersonal and collaboration skills.
  • Proficiency in assessing students' transition needs and developing goals and objectives to address those needs.
  • Effective management of caseloads.
Salary: As per the applicable contract.
