Position Summary

The Case Manager is an essential member of the social service team at St. Joseph Center. This role is pivotal in maintaining the organization's mission by providing high-quality case management services to families experiencing homelessness, ensuring they receive critical service connections and housing support.

Key Responsibilities

  • Collaborate with the Program Manager and the case management team to evaluate families for permanent housing solutions.
  • Oversee a caseload of up to 30 families, creating individualized case management strategies for each.
  • Facilitate referrals to necessary services, including healthcare, mental health resources, education, and employment opportunities, serving as an advocate when needed.
  • Work in conjunction with housing specialists to assist families in locating appropriate housing, negotiating rental agreements, and providing ongoing support after placement.
  • Provide continuous case management services to families following their placement.

Qualifications

  • Must be proactive and driven, with strong communication skills to engage effectively with a diverse range of individuals, including clients, community members, and colleagues. Excellent organizational and administrative skills are essential.
  • Demonstrated understanding of case management principles and intervention strategies relevant to homeless populations or other marginalized groups.
  • Familiarity with housing programs and the housing market is beneficial.
  • Ability to work flexible hours, including early mornings and evenings, is required.
  • Fluency in both Spanish and English is a plus.

Experience

  • One to two years of experience in providing case management services and advocacy for homeless and low-income families.

Education

  • Bachelor's degree from an accredited institution in Psychology, Social Work, Human Services, or a related field.

Direct Reports

  • This position does not entail direct supervisory responsibilities.

Technical Skills

  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook).
  • Ability to navigate common internet browsers.

Certificates, Licenses, and Registrations

  • Valid California Class C Driver License or reliable transportation options when necessary for job duties.
  • Current automobile liability insurance.

Travel Requirements

  • Travel to and from the workplace and other appointments as required.

Work Environment

Physical Demands
