Position Overview:

Under the supervision of the Tenant Services Supervisor I/II, the Case Manager I (CM-I) plays a crucial role in delivering personalized case management services to individuals and families who have previously experienced homelessness. Utilizing a theoretical framework that includes Motivational Interviewing (MI), Critical Time Intervention (CTI), and Stages of Change (SoC), the CM-I is equipped with effective strategies to encourage tenants in achieving their personal goals in a supportive and non-judgmental environment. The CM-I will engage in various specialized tasks that encompass comprehensive case management services such as intake, assessment, goal setting, monitoring, life skills training, counseling, assistance with benefits, and referrals for all tenants. Documentation of all tenant interactions will be maintained and entered into the Homeless Management Information System (HMIS) database.

Key Responsibilities:

  • Deliver individualized case management referrals and supportive services to tenants.
  • Facilitate group supportive services for tenants.
  • Ensure accurate documentation and maintain up-to-date case notes within a computerized database system (HMIS).
  • Process housing applications via the Coordinated Entry System (CES) and conduct interviews with potential tenants for permanent supportive housing.
  • Provide crisis management support to enhance tenant housing stability, increase income, and foster community involvement.
  • Offer coverage for programs that may occur during evening and weekend hours.
  • Act as a mandated reporter for any suspected abuse, ensuring immediate reporting of any concerns regarding violations of a person's rights.
  • Present tenant case reviews to the Tenant Services Supervisor on a bi-weekly basis.

Qualifications:

To succeed in this role, the Case Manager I must possess:

Basic Qualifications:

  • Bachelor's degree in a Social Services field.
  • Two (2) years of experience in case management or client advocacy services for individuals who have experienced homelessness and/or mental illness.
  • One (1) year of experience using a client tracking database system.
  • Six (6) months of experience implementing evidence-based practices in a social services context.
  • Valid California driver's license.
  • Access to a personal vehicle for conducting ACOF business.
  • Ability to meet California minimum and ACOF insurance requirements.

Preferred Qualifications:

  • Bilingual proficiency (English/Spanish).
  • Experience in providing basic case management approaches in permanent supportive housing for individuals and/or families facing mental health challenges.
  • Demonstrated ability to develop and facilitate life skills training groups.
  • Familiarity with the HMIS system.
  • Knowledge of the Coordinated Entry System and familiarity with the VI-SPDAT.
  • Capability to deliver crisis management services in challenging situations.
  • Proficiency in Microsoft Office applications (Word, Excel, PowerPoint, Outlook).
  • Strong keyboarding skills for producing correspondence, emails, and reports.
  • Effective communication skills in both verbal and written formats.
  • Awareness and appreciation of diverse tenant populations to effectively promote community and independent living skills.
  • Extensive knowledge of team dynamics and the ability to navigate various social service systems.
  • Capacity to provide non-judgmental support and guidance to individuals and families in stressful circumstances.
  • Experience in a collaborative team-oriented environment.
