Position Overview:

Under the supervision of the Tenant Services Supervisor I/II, the Case Manager I (CM-I) plays a crucial role in delivering personalized case management services to individuals and families who have experienced homelessness. Utilizing a theoretical framework that includes Motivational Interviewing (MI), Critical Time Intervention (CTI), and Stages of Change (SoC), the CM-I is equipped with effective strategies to inspire tenants in a supportive and non-judgmental environment. Key responsibilities include comprehensive case management tasks such as intake, assessment, goal setting, monitoring, life skills training, counseling, and facilitating access to benefits and resources.

Key Responsibilities:

  • Deliver individualized case management and supportive services tailored to tenant needs.
  • Organize and facilitate group support sessions for tenants.
  • Maintain accurate documentation and case notes within a computerized database (HMIS).
  • Process housing applications through the Coordinated Entry System (CES) and conduct interviews with prospective tenants.
  • Provide crisis intervention services to enhance tenant housing stability, increase income, and foster community involvement.
  • Offer program coverage during occasional evening and weekend hours.
  • Act as a mandated reporter for any suspected abuse or rights violations.
  • Present tenant case reviews to the Tenant Services Supervisor on a bi-weekly basis.

Qualifications:

To succeed in this role, the Case Manager I should possess:

Essential Qualifications:

  • Bachelor's degree in a Social Services field.
  • Two years of experience in case management or client advocacy, particularly with individuals experiencing homelessness or mental health issues.
  • One year of experience using a client tracking database.
  • Six months of experience applying evidence-based practices in social services.
  • Valid California driver's license and access to a personal vehicle for work-related duties.
  • Ability to meet California minimum and ACOF insurance requirements.

Preferred Qualifications:

  • Bilingual proficiency in English and Spanish.
  • Experience in providing case management in permanent supportive housing settings for individuals and families with mental health diagnoses.
  • Demonstrated ability to develop and lead life skills training groups.
  • Familiarity with the HMIS system and knowledge of the Coordinated Entry System, including the VI-SPDAT.
  • Capability to deliver crisis management services in challenging situations.
  • Proficiency in Microsoft Office applications (Word, Excel, PowerPoint, Outlook).
  • Strong keyboarding skills for effective communication and reporting.
  • Excellent verbal and written communication skills.
  • Awareness and sensitivity to diverse tenant populations to promote community and independent living skills.
  • Comprehensive understanding of team dynamics and the ability to navigate various social service systems.
  • Capacity to provide empathetic support and guidance to individuals and families in distress.
  • Experience in fostering a collaborative team environment.
