POSITION SUMMARY

The Housing Stability Coordinator will oversee the provision of integrated Case Management and Support Services for individuals transitioning from homelessness, particularly those with unique needs. This role involves delivering on-site assistance to residents at a newly established multi-unit permanent supportive housing facility. The Coordinator will facilitate connections to various services, including life skills training, financial management, educational opportunities, health care access, counseling, job training, and community engagement activities.

KEY RESPONSIBILITIES

  • Conduct initial housing assessments and intake interviews with each resident.
  • Assist residents in setting personal objectives and formulating plans to achieve these goals through the Individual Service Plan (ISP).
  • Address any existing challenges or barriers to ensure timely completion of housing-related processes.
  • Maintain regular contact with residents, ensuring a minimum of two face-to-face meetings each month.
  • Facilitate quarterly evaluations with residents to review progress and achievements based on Individualized Stability Plans.
  • Act as a communication bridge between the Property Manager, management team, residents, and primary mental health service providers to address any concerns that may impact residency.
  • Provide guidance on Fair Housing regulations, tenant rights, and responsibilities, as well as adherence to rental agreements and property policies.
  • Coordinate access to services from various public social service agencies and community organizations, supporting residents in achieving independence through employment and educational initiatives.
  • Assist residents in securing rental assistance and eviction prevention services.
  • Manage tenant complaints, mediate disputes, and resolve issues with property management that could affect tenancy.
  • Collaborate with property management to enforce housing regulations.
  • Work alongside the Resident Services Coordinator to create workshops on essential topics such as household management, financial literacy, parenting, and skills necessary for maintaining housing and achieving self-sufficiency.
  • Document client progress through data entry into the DHS CHAMP database, generating reports for program evaluation.
  • Organize and maintain comprehensive client files.
  • Monitor the status of tenants who have transitioned into housing and prepare those ready for movement to mainstream housing options.
  • Adhere to documentation and reporting requirements as outlined in service contracts.
  • Compile and submit monthly reports tracking residents' status and the supportive services provided.
  • Uphold the mission and strategic objectives of the Coalition for Responsible Community Development.

QUALIFICATIONS

  • 1-2 years of relevant experience and a Bachelor's Degree.
  • Experience working with youth facing homelessness, housing instability, or involvement in foster care, probation, or mental health systems.
  • Dependable, adaptable team player with a strong work ethic and the ability to maintain professional boundaries.
  • Valid driver's license and reliable transportation, including car insurance as mandated by law.
  • Proficiency in Microsoft Office Suite is required.

SALARY AND BENEFITS

The annual salary for this position ranges from $48,000 to $55,000. The Coalition for Responsible Community Development offers a competitive benefits package, including:
